By signing this application form, I confirm I wish to enter run4alife
and undertake to be bound by the event rules that are available at www.run4alife.com

I understand that I should seek medical advice from my general practitioner
if I am in any doubt as to my physical ability to take part in this
event.
I am entering this event entirely at my own risk, Wigan Rotary Club
shall not be liable for any injury or loss that might occur as a result
in my participation other than as a result of Wigan Rotary Club’s
negligence.
I understand that Wigan Rotary Club may distribute details and/or
images of participants to those providing media coverage of the event
and I consent to my name and/or image being passed to them and published
in any media reporting details of the event.
